Gulden's patented new Mini-Illuminated Near Card (Mini-INC) is a compact potential acuity meter allowing clinicians to accurately establish the retinal acuity in the office or at the bedside. The test is useful in predicting postoperative acuity prior to cataract surgery or YAG laser capsulotomy (predicts postoperative acuity to within 1 line in 97% of eyes when preoperative acuity is better than 20/200). The Mini-INC is valuable in guiding the surgeon in deciding the treatment option in eyes with cloudy media and macular disease. A clinician is now able to determine what portion of the vision loss is due to cataract and what portion is due to macular dysfunction. This is particularly helpful in patients with diabetic maculopathy, macular degeneration and other diseases affecting central vision. This pocket size device is indispensable for determining the retinal acuity behind cataract or in other causes of cloudy media.
In eyes with clear media, the Mini-INC is a time saving device for determining the best visual acuity for medical purposes without refraction. Also, it can quickly confirm that the best visual acuity has been achieved by refraction.
Technical
The Mini-INC combines three basic principles: visual angle, brilliant illumination, and small pinhole aperture. The small aperture pinhole increases resolution by encouraging alignment of the visual axis through a clear portion of the cataract or thickened posterior capsule and reduces optical aberrations by eliminating peripheral rays. The brilliantly lighted letters compensate for the reduced illumination resulting from viewing through a small pinhole aperture. The letters at 40cm subtend the same visual angle as Snellen letters at 20 feet. There are 9 lines of letters: 20/20, 20/25, 20/30, 20/40, 20/50, 20/60, 20/80, 20/100 and 20/200.
